Note that HIPAA does not automatically apply to an entity or person just because health information is involved, and it may not cover your transactions or communications with Vitality Health PLLC, the Medical Groups, the Providers, the Labs, or the Pharmacies.

If authorities deem Vitality Health PLLC a “business associate,” then, acting solely in that role, Vitality Health PLLC must follow certain HIPAA provisions regarding the “protected health information” (PHI) you provide to Vitality Health PLLC, the Medical Group, or the Providers. Additionally, we will use and disclose any medical or health information you provide—covered by specific protections under state laws (together with PHI called “Protected Information”)—only according to those applicable laws.

The Medical Groups and Providers have adopted a Notice of Privacy Practices that describes how they use and disclose Protected Information. By accessing or using any part of the Service, you acknowledge receipt of the Notice of Privacy Practices from your Medical Group and Provider(s).
